New Patients
We accept patients on to our list if they reside within the practice area.
You can either complete the registration form on line,
or you can collect a registration welcome pack at the surgery.
There is also the Family Doctor Services Registration Form – GMS1
and the Alcohol Data Sharing Questionnaire which you may be asked to fill in.
Identification Requirements
Any patient wishing to register with this practice, either temporarily or permanently, may have to provide two forms of identification.
- One to verify your registration address.
- The other has to be photo identification.
Any of the following can be used as identification:-
- Passport
- Driving Licence
- National Insurance Card P NHS Card
- Current Utility Bill/Bank Statement
- Birth Certificate
- Government Document
Change of Personal Details
To keep our contact records up-to-date, please inform us regularly of any changes to your address or telephone number and/or next of kin details. Please help us to help you.
