Access to Patient Information and Patient Confidentiality

This practice is registered under the Data Protection Act of 1998.

Our Practice GPs, nursing staff and administrative staff have access to both your written and computerised medical records. They are all governed by the same professional codes of confidentiality and will not disclose information without your consent. We will normally share some information with other health professionals involved in your health care, these could include doctors, nurses, therapists, pharmacists and technicians involved in your investigation or treatment. These professionals may work for a variety of organisations including local community NHS trusts, the PCT (Primary Care Trusts), social services or private hospitals. Law courts can also require GPs to disclose medical records to them. We are also often asked for reports from solicitors, these will always be accompanied by the patient's written consent for us to disclose information. You should also be aware that in certain circumstances your right to see some details in your health records may be limited in your own interest or for other reasons.

We are required by law to allow you access to your medical record. All such requests must be made in writing. We are allowed to charge a fee to cover our administration and photocopying charges.
